targgart Welcome! Thank you for visiting the Perry-Worth Elementary website! We are a rural community located on the southeast side of Lebanon. Our school is one of four elementary schools in the Lebanon Community School Corporation. Perry-Worth accommodates approximately 320 students in grades K-5. We offer a variety of support systems and services to accommodate a variety of student needs, including Special Education Service, Response to Intervention, Speech Therapy, and ENL. At Perry-Worth, student achievement is top priority as we continually strive to meet the needs of all individuals. We are dedicated to providing a safe and caring learning environment for all students and staff. With the appropriate encouragement, guidance, and leadership, we believe that each child has the ability to reach his/her own fullest potential. Before You Shop (Dress Code)
To help you shop for your Perry-Worth student, we have given you our DRESS CODE page out of the Elementary Handbook. Please read carefully. Policy

Transportation Guidelines
LCSC Transportation guidelines are in the attached PDF. Please note that students may only have ONE dismissal bus. Any changes made must be permanent.

Also, please note that any daily transportation change must be made before 3:00 p.m. by contacting your student's teacher and emailing the office at rileyk@leb.k12.in.us. Transportation

Volunteers Wanted!
We are looking for office volunteers! If you have some extra time on your hands and would like to help teachers at Perry-Worth with making copies, laminating, putting up or taking down bulletin boards, we would love to have you. Just call the main office at 317-769-3286 for details.

You must have a Limited Criminal History form on file each school year.

Child Find helps locate, and evaluate children and youth ages 3 to 22 years of age who are suspected of having or have a diagnosed disability or developmental delay, in order to provide appropriate special education services under the law. Click here for more information...

Private School Invitation to Consult on Federal Programs and Determination of Equitable Services
On behalf of the six (6) participating school districts, Boone-Clinton-North West Hendricks Joint Services invites representatives from your private school/agency to a consultation meeting regarding the design and development of special education and related services for students with disabilities within the districts of Lebanon, Clinton Prairie, Clinton Central, Rossville, Western Boone and North West Hendricks. Under the 2004 Amendments to IDEA, we are required to invite you to participate in this annual consultation meeting where we will discuss the extent to which special education services will be offered to students who are unilaterally enrolled by their parents in your private school or facility.
