Important Athletic Announcements

Athletic Forms and Resources

All athletes will need a "Tiger Card" to participate in athletics. This card will be given by the athlete to the coach prior to participation and will be good for the school year.

To receive this card all athletes need:
1. Physical Form (Two Pages)
2. Signed insurance waiver release
3. Signed Financial Obligation Form
Forms are below, print off, fill out, and turn in the first day of call-outs.

-Tiger Card Athletic Eligibility Form / IHSAA PRE-PARTICIPATION PHYSICAL EVALUATION
-Concussion Acknowledgement and Signature form for Parents and Student Athletes
